Job Summary:

In this HIV Medical Case Manager role, you will be an essential element of an integrated care team that recruits, assesses and qualifies patients for case management services at Kumukahi Health + Wellness.

 

Your primary role include, carrying a caseload of HIV positive clients possibly with medical comorbidities, qualifying patients for Ryan White services, creating and updating acuity and treatment plans, and monitoring HIV labs, including viral load. Case management services may entail securing medical transportation to/from appointments, assisting patients towards treatment readiness, facilitating telehealth visits, and other activities that ensure treatment successes.

Duties & Responsibilities:

  • Conduct comprehensive assessments of clients’ medical, psychosocial, and support needs.

  • Develop and implement individualized care plans in collaboration with clients, healthcare providers, and support services.

  • Coordinate and monitor access to medical care, medications, mental health services, substance use treatment, and other supportive services.

  • Advocate for clients within the healthcare system and with other service providers.

  • Provide education and counseling to clients about HIV/AIDS, treatment options, medication adherence, and healthy lifestyle choices.

  • Facilitate referrals to community resources and support networks.

  • Maintain accurate and up-to-date documentation of client interactions, care plans, and progress notes.

  • Participate in multidisciplinary team meetings to review and coordinate client care.

  • Conduct follow-up with clients to ensure continuity of care and address any barriers to accessing services.

  • Stay informed about the latest developments in HIV/AIDS treatment and care.

Abilities & Experience:

  • Bachelor’s degree in Social Work, Nursing, Public Health, or a related field, or a minimum of 2 years of experience in case management, preferably with individuals living with chronic or communicable diseases.

  • Basic knowledge of HIV/AIDS, including transmission, prevention, and treatment.

  • Understanding of Hawai'i Island geography and its unique populations, especially West Hawai'i

  • Must be able to communicate and function productively on an interdisciplinary team of various educational and professional backgrounds

  • Must provide services to patients in a manner that is non-judgmental, client-centered and strengths-based

  • Proficiency in data management and electronic health records.

  • Must be comfortable with Harm Reduction in underserved populations, such as sex workers, people without housing, and patients who actively use illegal substances

  • Experience working with LGBTQ and/or HIV individuals preferred

  • Experience in arranging community resources

  • Familiarity with medical insurance in Hawai'i

  • Ability to maintain the highest standards of patient confidentiality due to the sensitive nature of our patient base

  • Valid Hawai'i Driver’s License,

  • Proof of no-fault auto insurance and reliable transportation

Computer Skills:

To perform this job successfully, an individual should have knowledge of the Google suite of office tools: Google Docs, Sheets, Slides, Forms, etc as well as Microsoft Office, and Internet software as well as some Social Media knowledge.

Measures are taken to train staff to appropriately deal with these situations, but those in the position should reasonably expect these types of things to occur and your supervisor must provide appropriate direction and support to these situations.
